1 Overview
According to the latest projections, Hong Kong's population will increase from 7.24 million to a peak of 8.22 million in mid-2043, and then decline to 7.81 million in mid-2064. The proportion of elderly people aged 65 and above is expected to rise to 33 per cent in 2064, while it was 15 per cent in 2014.
Facing the challenges imposed on society by an ageing population, the Government has taken various measures to encourage active ageing. Yet, there is no guiding policy for active ageing in Hong Kong. Other Asian economics such as Japan, South Korea, Malaysia and Singapore have promoted active ageing to optimize opportunities for health, participation and security of the elderly in order to enhance the quality of life as people age.
The programme aims to promote health and wellbeing, and develop an awareness of healthy and active ageing in Hong Kong society. Health is a means to successful ageing, but not an end in itself and thus many other factors such as 'mental psychological health' and 'social well-being' are also linked to successful ageing. Concerning biological, mental psychological, nutritional and social factors contributing to healthy ageing, the holistic approach of the programme should enable participants to deliver suitable health planning and promotion programmes in the community for specific target groups.

2 Expected Programme Learning Outcomes
Upon completion of the programme, students should be able to
i) explain the biological, psychological, nutritional and social factors influencing ageing independence;
ii) advise individuals on effective strategies for maintaining physical and mental health, and social well-being;
iii) devise and deliver health plans and health promotion programmes in specific settings in an ethically responsible and professional manner.
Duration:The whole programme will normally last for 12 months (including lectures and practicum)

3 Programme Structure
The programme consists of 5 courses including face-to-face lectures and practicum.

5 | Practicum (4 full days) | In this course, students are assigned to CADENZA to take part in health promotion programmes related to successful ageing. They will put what they have learnt in other courses of the programme into practice. Students will spend 4 full days in the practicum venue, doing observation first, then delivering interventions and programmes to promote health. | 28 |

4 Target Participants
Elderly services providers, programme staff of elderly community centres, junior social workers and people working in the human resources sector would find the programme most beneficial to their work.
Helping professionals such as counsellors, life coaches, personal care workers, and individuals who are interested in health planning and promoting health in their own specific settings would also find the programme useful.

5 Award / Graduation Requirements
To qualify for the award of the 'Certificate Programme in Health Planning for Positive Ageing', students are required to:
1. have an attendance rate of 70% or above in each course
2. have passed the assessments of each course
** Registered nurses and enrolled nurses under the Nursing Council of Hong Kong will receive Continuing Nursing Education points (CNE points) after completing this programme.
** Registered Chinese medicine practitioners under the Chinese Medicine Council of Hong Kong will receive Continuing Education in Chinese Medicine points (CME points) after completing this programme.
** Registered physiotherapists under the Physiotherapists Board, who complete this programme and have an attendance rate of 75% or above in each module, will receive CPD points for Physiotherapists [CPD(PT) points].

6 Joint organizer
Jockey Club CADENZA Hub ('JCCH') is one of the innovative community projects funded by The Hong Kong Jockey Club Charities Trust under 'CADENZA:A Jockey Club initiative for Seniors' aiming at improving the quality of care and quality of life for older persons in Hong Kong. JCCH is a one-stop integrated health and social care centre headed by Professor Jean Woo from the Chinese University of Hong Kong and a team of multi-disciplinary healthcare professionals. In the past six years, JCCH has organized various primary health services and programmes, e.g. health workshops and talks, exercise class, and health path projects to the community. Near 20,000 attendances have been reached.
